Appointments
Our office is open Monday, Tuesday and Thursday from 8:00 AM to 4:30 PM. Occasionally we are open Wednesdays from 8:00 AM to 12 noon. We do require a deposit for chair time after two missed appointments. Appointments
are scheduled to respect your time and ours. Please give us a 48 hour
notice if you must change your appointment. We will make every attempt
to give you a courtesy phone call two days before your scheduled appointment.

For Your Health and Safety
This office adheres to strict OSHA guidelines for your protection and
ours. We have the most up-to-date sterilization equipment and monitoring
devices. We use disposable dental supplies and all rooms are disinfected
prior to each patient's treatment.

Your First Visit
Your first visit will consist of a thorough exam, cleaning and consultation.
This may include a series of x-rays and supporting diagnostic data followed
by an explanation of our findings. Once a treatment plan has been established,
your appointment sequence will be arranged. Treatment alternatives and
financial arrangements will be discussed at this time.

Insurance
We will be glad to assist you in obtaining the maximum benefit from your
dental insurance. You will be responsible for any charges not covered
by your dental plan. Please be prepared to pay your portion the day of
service and bring a current insurance card, claim form, or booklet from
your employer. We are providers for Blue Cross Blue Shield PPO, Delta Dental. Health Partners and Metlife. We will file on most insurance plans. Please pring all insurance cards and information.

Emergencies
We will make every effort to see any true dental emergency. Please call
our office as early in the morning as possible or leave a message on our
answering machine. We do not call in any prescriptions unless a patient is currently under our care for current treatment. We only provide after hours emergency care for our active patients of record.
